OCCUPATIONAL THERAPY IN FORENSIC MENTAL HEALTH

The Bombay Branch of A.I.O.T.A has organized a guest lecture for its members, on 'OCCUPATIONAL THERAPY IN FORENSIC MENTAL HEALTH' by Dr. Samita Kirve (Specialist Occupational Therapist at NHS, Harrow PCT, London) on Thursday 18th February 2010 at 1:30 pm. The venue of the lecture is 4th floor O.T  lecture hall, College building, L.T.M.M.C - Sion Hospital, Mumbai. All members are cordially invited.

HAEMOPHILIA-OCCUPATIONAL THERAPY MANAGEMENT



Haemophilia is a hereditary disorder of coagulation that results in deficiency of Factor VIII, Factor IX OR Von Willebrandt factor. It is present almost exclusively in males and females are the “ASYMPTOMATIC CARRIERS”. Females may be symptomatic in very rare cases.

TYPES OF HAEMOPHILIA:       
Depending on the factor:
1]. Haemophilia A – deficiency of Factor VIII
2]. Haemophilia B – deficiency of Factor IX
3]. Haemophilia C – deficiency of Factor XI
4]. Von Willebrandt’s Disease[factor VIIIc] – deficiency of von willebrandt factor

Depending on level of factor:
Normal factor levels are 50-150
Severe Haemophilia – Factor level <1%
Moderate Haemophilia – Factor level 2-5%
Mild Haemophilia – Factor level 5-25%

SYMPTOMS:
-Recurrent spontaneous bleeds especially in major joints of the body i.e knee,ankle,elbow etc.
-Recurrent spontaneous bleeds in muscles.
-Excessive bleeding on trivial trauma that takes long time to subside
-Red hot swollen joint with a feeling of tightness around the joint
-Unable to move the affected joint due to pain and tightness

MEDICAL TREATMENT:

  • Factor infusion for replacement of deficient factor.
  • Analgesics or pain killers to reduce pain.


OCCUPATIONAL THERAPY MANAGEMENT:
  • Icing to acute swollen joint.
  • Ultrasound to facilitate early healing.
  • Early gentle joint ROM exercises under supervision and with assistance. Active assistive resistive exercises to develop and maintain muscle power.
  • Traction to maintain joint space and allow healing.
  • Use of protective cuffs to prevent further bleeding episodes due to trivial trauma to the joint.
  • Prescription and monitoring therapeutic exercises such as cycling, swimming etc.
  • Educating about joint protection techniques.
